You would have looted the Family Caudex Insignia off Gati the Storm-Sister. Taking it or activating it in your inventory would have started the quest.

On the body of the Dremora, you find a half-melted house insignia bearing the inscription "T. Caudex." It appears to be a part of a medal or memento of some sort.
